The State offers dental benefits through Blue Cross of Idaho to eligible employees and their eligible family members. You are automatically eligible for the plan if you are enrolled in one of the State's medical plans. Note: Dental is available only if you participate in a State employee medical plan -- you can't elect dental coverage without electing medical insurance. For premiums and full details see:
The State offers vision benefits through Vision Service Plan. You can use any provider you want; however, VSP providers offer discounts and will submit billing to VSP on your behalf. To find participating vision providers, please contact Vision Service Plan, 1-800-877-7195 or go on-line to , on the MEMBERS tab select "FIND A DOCTOR". If you choose to see an out-of-network provider, visit the Benefits & Claims section on the . In order to log into the VSP website, you will need to enter your 9 digit member identification which is also your Blue Cross enrollee identification number, found on your Blue Cross ID card. After your eligibility for group health care coverage ends, you may be able to purchase continued medical and dental coverage on an individual basis, for a period of time under the Consolidated Omnibus Budget Reconciliation Act of 1985, (COBRA). Employee Assistance Program (EAP)

Through the program, there are services for emotional, behavioral, mental health and substance abuse issues for each individual. These sessions have no out-of-pocket co-payments and do not count against the available mental health and substance abuse services. EAP benefits must be pre- authorized through ComPsych. Accidental Death and Dismemberment
一道本不卡免费高清The Accidental Death and Dismemberment (AD&D) benefit covers all ISU benefit, eligible employees. AD&D pays a percentage of annual salary for certain serious physical losses, including loss of life, due to a covered accident. For full details see:
When you start work as a benefit eligible employee, you will need to complete the appropriate new hire packet in Human Resources. After your initial enrollment orientation, electing changes can be made with paper applications in Human Resources or by printing the appropriate Forms and then submitting them to HR.
Insurance Effective Date
Insurance benefits will take effect on the first of the month following their start date. The deductible year coincides with ISU's fiscal year, which is from July 1 to June 30. For full details see:
When Coverage Ends
